Requisition ID: xxxxLocation: PLC Cor Mariae CenterLocation Address:xxxx Maria Linden Drive, Rockford, IL xxxx4 United States (US) Daily Hours: 0Standard Hours: 0Employment Status: PRN / Registry Employment Type: Regular Shift: RotatingFLSA: E SUMMARY Provides direct patient care to include; speech, language, and swallowing therapy to patients upon physician referral in accordance with all laws, regulations, and standards. ESSENTIAL DUTIES AND RESPONSIBILITIES Involved in community health matters/projects as appropriateMust work under the direct supervision of a licensed SLP-CCC or SLP-CFYServes as a liaison to patients, families, support departments, etc., to adequately plan for patient needsPossess the ability to make independent decisions using good reasoning skills and judgement when circumstances warrant such actionKnowledgeable of rehab practices and procedures, as well as the laws, regulations, and guidelines governing rehab functions in the long-term care facilityPossess willingness to work harmoniously with professional and non-professional personnelAbility to implement company policies, programs, goals, objectives, including procedures of the Rehab departmentAbility to assist in the evacuation of patientsExercise good body mechanics at all timesAbility to establish, assess, and modify realistic, measureable, timely, and functional goalsAbility  to oversee and evaluate care given by SLP-CFYs and Rehab AidesAbility  to follow speech therapy treatment plans for patient under direction of the supervising CCC/SLPAbility to document therapy evaluations and treatments accurately and timelyMust be able to utilize therapy software and equipment appropriately and accuratelyMust be able to read, write, speak, and understand the English languageAbility  to communicate and function in an interdisciplinary team SUPERVISORY RESPONSIBILITIES                                                                                            Does Not Supervise Employees Directly supervises employee(s).
